Hammersmith and Fulham Designated Worker – QPR in the Community Trust

Job Description
Reporting to the Youth & Communities Manager (and to a named manager at Hammersmith and Fulham YOT).
The postholder will be responsible for supporting the delivery of the work of the Youth and Communities Team for QPR in the Community Trust with a focus on Hammersmith and Fulham. Under the guidance of the Prevent and Prosper team will liaise with referral partners and relevant organisations to support the Prevent and Prosper project.
To work alongside YOT staff to, develop, and support young people engaged with the Prevent and Prosper project and achieve a range of positive outcomes in their lives, including recidivism, employability, training and education outcomes.

Key Responsibilities:
• To work as part of the YOT multi-disciplinary team.
• To design and deliver bespoke packages of support for young people that are subject to Out of Court Disposals (OOCDs), including diversion/Triage.
• To promote the other QPR opportunities to young people engaged with the Prevent and Prosper service
• To attend relevant meetings as required and guided by the Youth & Communities Manager
• Undertake written and other monitoring, recording and evaluation as directed by the Youth & Communities Manager.
• Effective networking and referral pathways with a range of statutory and voluntary agencies.
• Undertake any other duties or tasks, which may be reasonably considered within the remit of the post.

Skills
• Demonstrable ability to communicate and engage with a wide and diverse range of young people face to face, one to one and in a group
• Empathy with young people engaged with the criminal justice system
• Ability to think creatively to source a variety of options that will support the progression of you people
• Establishing positive relationships with vulnerable young people.
• Ability to work as part of a team, and on own initiative.
In addition to the above skills the ideal candidate will possess strong communication and organisational skills and effective time management.

• Local young people from catchment boroughs, including care-experienced young people who may be disadvantaged in a number of ways, including: not being engaged in education, employment or training, having low confidence, lacking family support, being young carers, abusing drugs or alcohol, being young offenders, being isolated, being victims of bullying.

Experience
• Experience of partnership working within youth justice/criminal justice or school referral fields. (Essential)
• Ability to build good working relationships with vulnerable young people. (Essential)
• Experience of working with vulnerable young people. (Essential)
• Experience of providing support for disadvantaged young people. (Essential)
• Experience of working with Statutory and Voluntary Sector organisations. (Desirable)
• Ability to develop creative solutions to complex problems
• Level 1 Youth work or above (Desirable)
• Some experience of managing a referral-based project.
The successful candidate will be required to undergo a Disclosure & Barring Service Check (DBS).
