An opportunity has arisen in the Health and Physical Activity Team to recruit a Physical Activity Referral Co-ordinator (37 per week).

As the Physical Activity Referral Co-ordinator you will be responsible for the ‘Active Health’ Physical Activity Referral Scheme, which includes a number of specialist areas; falls prevention, exercise after stroke, cardiac rehabilitation and long term conditions. Whilst reporting to the Health and Physical Activity Manager you will work in close partnership with Public Health colleagues.
About the role
You will have experience of delivering exercise to people with a range of specific medical conditions in order to improve health outcomes and you will have a sound knowledge of the exercise industry, together with experience of managing and delivering exercise referral.
About you
Qualified to REP’s Level 4, with an Exercise Referral Instructor qualification and either a BACPR, Postural Stability, or Exercise After Stroke qualification, you will be highly motivated, have excellent communication and inter-personal skills, be able to work as part of a team and on your own initiative. You will be highly organised and have the ability to develop and drive programmes forwards and work under pressure ensuring key priorities and deadlines are met. Good IT skills are essential, as part of the role is the management of our online referral tool.
You will work with and support the Physical Activity Referral Hub-Co-ordinators with the delivery of the county-wide physical activity referral scheme, ensuring that key processes and procedures are followed. You will be responsible for marketing and promoting the scheme with GPs and other health professionals, ensuring that patients are referred to appropriate exercise opportunities. You will also support the Health and Physical Activity Manager with the co-ordination of the specialist exercise classes. Knowledge and experience of setting and managing performance indicators, both qualitative and quantitative and monitoring and evaluation of projects is essential.
The successful applicant will also be responsible for advising and guiding exercise instructors as required. This will include the provision of Continuous Professional Development opportunities and therefore the ability to tutor would be beneficial.
You will be expected to demonstrate the full range of competencies required of a Council Officer in a rapidly changing, performance driven environment. You must hold a current, valid driving license and have a vehicle available for work purposes.
